Individual injector drivers a la sequential injection? Otherwise, it would have to be the injector, no?

Semi sequential. Each injector is on it's own driver. But it's not timed with the intake valve since there is no cam sensor.

Well so much for the mystery... just got a txt from my buddy... he was putting the injector clips back on and this is what happened to cyl # 6 injector:

Well the wire wasnt out all the way it was loose in the connector (the bare one).
Because the wires were taped together the wire wasn't able to come out completely.
I wish the wire would have come off completely.

Curious, what size intercooler are you running? has it run enough to notice any heat soak issues?

I don't recall the size it was orginally, but it is a Precision core that has 4 rows chopped off (they were mangled from a nother inciedent), but no signs of out of control IAT's.. It's a really good core, and still not very small even with the missing rows.
