so my jack pads have finally rusted through...
anyone have any solutions?

Cut them out. weld in new 3x3 squared pipe.

is there a diagram of the rockers and the jack pads.. or could someone describe exactly what i need to cut and weld.

Doesn't Pelican Parts sell some sort of adapter jack pad that plugs into your side skirt
