They spray every where they can so it will protect the floor, doors, rockers, trunk, etc.
And as for new cars I would suggest getting it done after 2 years. Every time you have the car up on a hoist you should check the factory undercoating for damage (snow and ice can scrape it off and leave the metal exposed to salt).
And for older cars or damaged undercoating....get this stuff (i'm pretty shure that this is OEM BMW undercoating).
