For the only position 4 issue, blower motor resistor. Around $80 at the dealer, pretty easy swap.
As for no heat, that's a bigger issue.

Clogged heater core, broken heater valve, clogged cabin filter, busted thermostat...

Resistor...for the 4 part, i got one used for $20 works on all 4.
The rest can be many things, headgasket beeing the most common one...(will get air in your cooling system, and the heater core will be empty and cold)

First thing to check would be your coolant level if you are not getting heat....if its low top it up...then if it goes down you have a problem..some of which are mentioned above..a clogged heater core can be a major one...you can flush it out by taking both lines off and putting a hose on one end and blowing water through it and then reversing the process..do this a few times and you can usually blow most of the junk out of it.

sorry a question....
what causes a dead fan ? all fuses are good.
it would die then come back for a few weeks, then die..
it hasn't worked in a few weeks now.
